GPCCMP

GPCCMP Medicare rebate – A GP Chronic Condition Management Plan (GPCCMP) is issued at the discretion of a GP. If you are entitled to receive a GPCCMP, you can claim a partial rebate for the nominated number of Speech Therapy sessions indicated by your GP (up to 5) from Medicare over a 12 month period. Your invoice must be paid in full to All Communication Matters first and then they will issue you a receipt eligible for claiming at Medicare.

Our clients are advised to make their claims directly through the Express Plus Medicare mobile application. This app can be downloaded for free through the Apple app store and Google Play.

If you are visiting the Hornsby clinic at the time of the appointment and have your Medicare card and a Cheque or Savings card for the refund to be processed to, then your refund can be processed in the clinic by All Communication Matters, on your behalf.

Please note that a GPCCMP cannot be used at the same time as private health insurance or NDIS, and claims can only be made for appointments on or after the date of the GP signature on the GPCCMP letter/form.
